Your day
As a Home Health LPN you need to know how to:
- Assist in collection of a holistic, age-specific admission history and physical assessment. Deliver nursing plan of care on all assigned patients.
- Perform phlebotomy procedures, collect specimens and assist in coordinating lab result reports and perform quality control checks
- Assist in scheduling tests ordered by the health care provider. Obtain appropriate treatment authorizations, schedule specialist appointments, call client companies, and schedule follow up appointments
- Assist with discharge activities including completing discharge documents, reviewing post visit instructions related to medications, activity, and wound care, and follow-up appointments and questions
- Promote mission, vision, and values of SCL Health, and abide by service behavior standards
Your experience
We hire people, not resumes. But we also expect excellence, which is why we require:
- Graduate of an accredited LPN program, required
- Current Colorado LPN license or ability to obtain, required
- Current BLS certification, required
- Demonstrated phlebotomy skills or successful completion of phlebotomy in-service from the American Heart Association, required
- At least six (6) months of LPN experience in a comparable setting, such as home health or acute care, highly preferred
- Current IV Certification or obtain IV Certification in the State of practice within 3 months of hire, required
