Performs duties in accordance with applicable state, federal and TJC regulations. Assures collaboration across the enterprise of Centura and is a service excellence champion to ensure the agencies are constantly striving to be the provider of choice.What You Bring/Job Requirements:In addition to bringing your whole self to the workplace each day, qualified candidates will need the following:  * Associate Degree PT, OT and MSW also accepted.* Bachelor required within 2 years of hire or transfer for Nurse candidates only.* 1 year of recent home health/hospice/palliative care. Requires the employee to demonstrate competency in the provision of individualized care of patients based on the assessed age and developmental needs.* BLS* Valid Colorado Driver's License Medium Work - exert/lift up to 50 lbs. force occasionally, and/or up to 20 lbs. frequently, and/or up to 10 lbs. constantly This rehab supervisor role will consider a candidate that has a PT, OT, or SLP license.  Your Connected Community:When people and places align, incredible things happen.
