What You Bring/Job Requirements:In addition to bringing your whole self to the workplace each day, qualified candidates will need the following:  6 months home care/hospice experience preferred. Two years post Master's experience in health care or related field preferred.MSW (Required)LCSW PreferredDriver's LicenseBasic Life Support (BLS) from American Heart Association required for facilities with an Automated External Defibrillator (AED) preferred for facilities without an AED. Cardiopulmonary Resuscitation (CPR) required  Medium Work - (Exert up to 50lbs force occasionally, and/or up to 20lbs frequently, and/or up to 10lbs constantly) This role will cover north and south Denver.  The shifts are Monday - Friday (8:00am- 5:00pm). Your Connected Community:When people and places align, incredible things happen.
