The Dementia Care Coordinator Registered Nurse is directly responsible to the Nurse Manager of the Community Living Center (CLC) at the Syracuse VA Medical Center. Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. Physical Requirements: The population of male and female Veterans served ranges from the adult age 18, to the elderly adult age 65 and over. Syracuse VAMC is a is a Level 1c major interdisciplinary teaching facility with service-lines serving medical, surgical, and neurological inpatients and outpatients, physical medicine and rehabilitation, oncology, dentistry, and geriatrics, with a Level III Emergency Department, and multiple community-based outpatient clinics (CBOC). There is a large outpatient psychiatric population, with admissions for medical co-morbidities. Home-Based Primary Care nursing care is also provided. They Syracuse VA provides primary and behavioral health care at its Community Based Outpatient Clinics in Binghamton and Rome and five contract outpatient clinics. This position requires visual acuity, keen hearing, clear distinctive speech, and manual dexterity. This position requires potentially long periods of continued walking, standing, stooping, sitting, bending, pulling, and pushing. Transferring patients and objects may be required. The incumbent may be exposed to infected patients and contaminated materials and may be required to don protective clothing in isolation situations or operative/invasive procedures. The incumbent may occasionally be exposed to patients who are combative secondary to delirium, dementia, or psychiatric disorders. The Dementia Care Coordinator is responsible for assisting with case management and inspections of contracted Adult Day Health Care (ADHC). The Dementia Care Coordinator is a core member of the Community Nursing Home program, as such performs all nursing inspections and audits of contracted Nursing Home facilities, and assists the Caregiver Support Program with initial and annual home visits. As a clinical resource to Primary Care Teams (PACT), Behavioral Health Care Teams (BHCT), and Geriatric and Extended Care (GEC) programs, the incumbent is responsible for coordinating care across the continuum of inpatient and outpatient geriatric and extended care services and will function as a liaison between the VA staff and the local community agencies. In addition, the incumbent will also act as a liaison between the GEC programs, Primary Care staff, Behavioral Health Care Team, and specialty clinics for a select group of patients.
