Seeking a full-time physician assistant or nurse practitioner with a minimum of 1-year of Geriatric experience to join Providence ElderPlace in Portland, Oregon. Here, you will work with a well-established team providing older adults comprehensive care and social services while also managing a small panel of patients at a primary practice location. • Outpatient primary care setting, home visits and visits in community-based settings
• Collaborate with physicians, nurse care manages, social workers, pharmacists, behaviorists, and other team members
• 2-years of post-grad clinical experience working with geriatric population or hospice population preferred
• At least 1 year of Geriatric experience required
