This recruitment for Nurse Practitioner (NP) is specifically targeted to NP applicants with experience in Geriatric Medicine and the provision of Hospice and Palliative Care to fill a clinical position with some administrated duties. The NP serves Veterans who may be elderly, infirmed or with potentially life limiting illness within the Geriatrics and Extended Care Service Line of the Salisbury VA Health Care System (SVAHCS) located in Salisbury, North Carolina. The Nurse Practitioner (NP) is credentialed as a licensed independent practitioner within the Geriatrics and Extended Care Service Line (GEC) of the Salisbury Health Care System (SVAHCS). The NP works within a group GEC practice and seeks advice, recommendations and/or direction from the collaborating physician(s) or consultants in the event of a complex or unfamiliar situation that arises outside their clinical purview with the except for the provision of emergency care. The NP will first establish the availability of a collaborating physician prior to rendering clinical care, which may be unfamiliar or outside of their clinical expertise. The NP reports directly to the Associate Chief of Staff for GEC (ACOS/GEC) or their designee. The Geriatrics and Extended Care Service (GEC) provides care to Veterans in several clinical venues including inpatient, outpatient, community, and research settings, including but not limited to: the Community Living Center (CLC), Hospice and Palliative Medicine (Hospice House and outpatient), Geriatric Evaluation and Management Clinic (GEM Clinic), Home Based Primary Care, Inpatient and Outpatient Consultation Services (Geriatric and Hospice Services), Community Outreach, Education and Research Services. Nurse Practitioners assigned to the GEC Service are expected to provide clinical care in all settings as required. The Nurse Practitioner is responsible and accountable for individual nursing practice and determines the appropriate delegation of tasks consistent with the NP's obligation to provide optimal care (ANA Code of Ethics for Nurses). The NP practices according to guidelines established for the specific practice area. The scope of practice for the NP includes prescribing medications and devices. The NP functions as an advocate for Veteran patients in meeting their unique physical and psychological needs. Other responsibilities and duties include but not limited to the following: Conducts clinical practice and professionalism as outlined within the Bylaws and Rules of the Medical Staff of the SVAHCS. Communicates and interacts appropriately and courteously with all internal and external customers. Maintains confidentiality of electronic, written, and/or verbal patient/employee information. Demonstrates working practices that include adherence to confidentiality, privacy, security, safety, and Infection Control. Controls standards along with safe use and operation of equipment. Demonstrates basic or advanced computer skills in using automated medical record systems. Leads and organizes delivery of care to assure continuity of care and peer accountability for practice, including access to care and discharge planning Uses sound clinical judgment in assessing, planning, implementing, documenting, and evaluating care, based on the ANA Standards of Nursing Practice. Anticipates and recognizes medical/surgical/psychiatric emergencies and initiates or leads others to implement appropriate and timely actions. Uses advanced clinical knowledge/judgment to promote staff involvement in planning, decision-making, and evaluating outcomes. Functions as an expert in clinical practice and/or areas related to the assigned roles and responsibilities. Systematically evaluates current practice and formulates outcomes for groups of Veteran patients and/or organizational processes within area of expertise. Guides, develops, and supports staff from a leadership perspective. Uses professional standards of care, scientific evidence, and practice to evaluate programs/service activities Performs other related duties as assigned within scope of practice.
