The Senior Social Worker is the mental health consultation-liaison for VANTHCS. The consultation-liaison ensures continuity of mental health care between the emergency department, medical inpatient services and mental health services of Veterans. In addition, depending on the circumstances the consultation-liaison ensures continuity of mental health care between VANTHCS and a variety of community locations outside of a VANTHCS setting. The consultation-liaison provides complex mental health assessments with independent diagnoses, intensive case management services and psychotherapy to Veterans to determine psychiatrist interventions and assignment to mental health services such as general psychiatric outpatient services, acute inpatient psychiatric services, substance use disorder outpatient services, substance use disorder acute inpatient services, opioid replacement services, geriatric outpatient services, geriatric acute inpatient services, domiciliary placement services, HUD/Veteran Affairs Supportive Housing (HUD/VASH) homeless services, Health Care for Homeless Veteran (HCHV) homeless services, Grant Per Diem (GPD) services, Trauma services, Intensive Community Mental Health Recovery (ICMHR) services or other mental health services offered by VANTHCS. The consultation- liaison provides on-going case management and early treatment interventions to the Veteran until the Veteran has transitions to a mental health program or determined not to require mental health services. The Senior Social Worker is an expert at the community resources available to the Veterans living with mental health illness and assists Veterans in linking them to supportive environments in the areas in which they live and/or assist VA social workers resources as needed. The Senior Social Worker will provide face-to-face evaluations. The Senior Social Worker will function as the liaison between the various VANTHCS medical service programs and the mental health service programs and ensures appropriate coordination of care, and transitions of care for enrolled Veterans. The Senior Social Worker will provide mental health psychotherapy groups in various locations as needed. Performs specialized treatment of complex physical or mental illness as it relates to mental health illness. Incorporates complex multiple causation in differential diagnosis and treatment of Veteran patients, including making psychosocial and psychiatric diagnoses within approved scope of practice. Expands clinical knowledge in the profession, demonstrating innovation in the creation of new models of psychosocial assessment or intervention to identify and address specialized clinical needs of mental health illness. Ability to write policies, procedures, and/or practice guidelines pertaining to the specialty population or specialty treatment program. Develops and implements methods for measuring effectiveness of social work practice and services, utilizing outcome evaluations and use of evidence-based treatment modalities that are effective with Veterans who struggle with mental health illness along with medically and secondary psychiatrically complex conditions. Incumbent also provides specialized assessments, and treatment planning, as well as mental health consult to other team members to provide interventions and strategies for engagement for difficult to engage clients. Independently conducts comprehensive mental health assessments and develops treatment plans in collaboration with Veteran, family, and interdisciplinary treatment team. Monitors e-consultations, complete mental health assessments, and make recommendations to the Consultation-Liaison Team Leader and other VANTHCS medical service programs in addition to provide direct clinical care to Veterans struggling with mental health concerns. Assists Veterans when appropriate to link them to community services. Develops and maintains effective partnerships with VANTHCS Emergency Department staff, medical service programs and other VANTHCS teams and programs and follows leadership guidelines regarding access to VA mental health programs within VISN 17, including use of VA contracted care. Routinely participates in all Consultation-Liaison meetings to ensure good transitions of care for population served. Develops and provides ongoing training and in-services to multidisciplinary team members in area of expertise. In addition to the specialty consultation liaison specific job duties listed above the consultation liaison is also required to perform the general social worker duties required as a clinician in the Mental Health Division.
