This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Learn more The General Caregiver Support Program Coordinator is a Social Worker whose primary responsibilities are to provide clinical evidence-based services and interventions; program development; caregiver, Veteran and staff education on caregiver issues; community outreach; resource development; continuous quality-improvement activities and evaluation/consultation. The Coordinator will function in support of caregivers, Veterans and family members throughout the Medical Center and in the community. Major duties include, but are not limited to: Provide screening, assessment, diagnosis, and treatment of depressive and anxiety-related disorders, as well as other Axis I conditions, with an emphasis on the application of time-limited, evidence-based approaches. Assess and provide treatments for sub clinical symptoms of dysphoria and anxiety, adjustment difficulties, and excessive use or misuse of alcohol or other drugs. Provide services to the families of Veterans with dementia being served by BRO Team,HBPC, Adult Day Health Care (ADHC), Dementia Chronic Disease Management Program, and Nursing Home Care Unit (NHCU), when such care is directly related to the overall treatment plan for the Veteran. Assess patients' understanding of advanced directives and other forms of consent and assist staff in determining how to present treatment options and other such material to patients in a format they can understand. Facilitate transition and adjustment to new living situations (e.g., nursing home, assisted living facility), and provide seamless intervention services, as appropriate. Responsible to provide evaluation to patients with dementia related symptoms and diagnoses as part of the Dementia Case Management Program and Geriatric Evaluation and Management (GEM) Clinic. Apply new approaches to patient care through integration of approved investigative and research methods in clinical practice, basing interventions on evidence of effectiveness.
