The Licensed Practical Nurse is responsible for providing Nursing Care to residents in Long Term care, dementia unit, respite, hospice, rehabilitation, transitional care and their families/significant others. Duties include, but aren't limited to: Pain management. Palliative Care. Geriatric Assessment Skills. Care of Dementia Patients. Physical Restraint Policy & Procedure. Discharge Planning and Teaching. Patient Wanderer Security System. Facilitate Inter-disciplinary Meetings. End of Life Care Planning. Rehabilitative Nursing. Interacting with other therapy modalities i.e. (RT, PT, OT, Restorative). Administering medications ordered by the provider in accordance with BCMA process . Completing treatments as ordered by the provider- works with wound care nurses as needed. Work Schedule: 3:45pm - 12:15am, with weekend/holiday rotation.
