The Registered Nurse (RN) in Primary Care is an RN whose primary role is that of a care manager and Patient Aligned Care Team (PACT) leader. The PACT RN responsibilities include: Clinical leader for the Primary Care PACT team Acting as a principal point of contact for patient access to the primary care team The RN coordinates care during transitions by collaborating with multidisciplinary services, both internal and external to VA, linking the patient with services, resources, and opportunities based on needs. The RN uses motivational interviewing to guide patients to establish goals. Identifies high risk populations/chronic disease states; performs scheduled clinic visits to identify educational needs; provides coaching and support of self-management, prevention, and wellness; and makes referrals, such as Home-based Primary Care (HBPC); Home Telehealth; Palliative Care; Hospice; etc., as appropriate. The RN collaborates with key members of expanded team, such as Behavioral Health, MOVE Coordinator, Women's Health, and others, as necessary. The PACT RN places orders according to protocols; triages/assesses walk-in visits; performs telephone visits as part of discharge follow-up; coordinates care, such as procedures; labs; teaching; visits; transitions; and other needs.
