HBPC Registered Nurse (RN) is responsible for providing in home patient centered care and case management to assigned patients. RN functions as team leader/charge nurse providing leadership that facilitates delivery of high-quality nursing care through the use of the nursing process. Works effectively with all levels of staff to include collaborative efforts with other health professionals. RN provides leadership in improving patient care, maintaining standards of care, and directing and teaching other personnel in a variety of settings. Is adept at consideration of patient's developmental stage which may be anywhere on the continuum of young adult to geriatric. RN is expected to comply with all mandated annual review requirements, rotate to other clinical areas as assigned and take responsibility for attending in-service and continuing education activities. Demonstrates nursing knowledge and clinical skills with sound nursing judgment and the ability to care for patients. Demonstrates ability to be flexible in schedule and assignments. RN must be poised and articulate in communicating with people of varied educational levels and varied ethnic backgrounds and willing to work cooperatively as a member of a team in all assignments. Duties include, but not limited to the following: Utilizes critical thinking skills and clinical decisional authority, prioritizes patient care by assessing the needs of the patient. Delivering direct nursing care in the home setting. Developing and coordinating the patient's plan of care to include ongoing assessment and re-assessment, care planning , care coordination, monitoring, and evaluating clinical outcomes. Interacting, coordinating and advocating for the patient/caregiver with other relevant parties across a continuum of care, beyond a single episode of care, ensuring seamless transitions. Providing individualized education and support to the patient and caregiver to optimize clinical outcomes and support quality of life decisions.
