This position is located within Telehealth, Geriatrics and Extended Care Service .Phoenix VA Health Care System (PVAHCS), Phoneix, AZ. PVAHCS is a Level 1a major interdisciplinary teaching facility with functional service lines serving medical, surgical, mental health, and long-term care lnpatients and outpatients with an Emergency Department and multiple community-based outpatient clinics (CBOC}. Major Duties: A Telehealth Physician Assistant (TH PA) provides professional assessment, medical management, and coordination of health care services. The TH PA acts on behalf of the veteran to ensure that appropriate clinical services are received and manages the patient's assigned Disease Management Protocol (DMP) to meet clinical goals. Cross coverage may include assisting the Telehealth NP position when staffing is limited. The HT PA will work closely with the telehealth NP position daily to address out of range metrics (red alerts in vendor's software) on assigned patients The TH PA will maintain a primary care scope of practice. In addition, the TH PA provides ongoing monitoring and evaluation of care management services for patients enrolled in Home Telehealth (HT}. The TH PA will assist the HT Team Lead in providing guidance to the other HT program team members and provides cross coverage needed. The HT PA will manage a panel of HT patients where the consulting provider has elected to· utilize a TH PA. The HT PA works closely with primary care providers, specialty clinics, HT team members, and community agencies. The HT PA will manage medications, labs, relevant imaging, and coordination of services for HT patients that are assigned to their panels based on the service identified in the consult request. The TH PA provides comprehensive in-home visits via clinical video technology (CVT) in collaboration with local fire departments. The TH NP reviews the facility's daily discharge list to determine appropriate patients for local fire department programs. The TH PA also conducts CVT visits with patients that are referred by the collaborating fire departments. Work Schedule: Monday to Friday 7am to 3:30pm. Financial Disclosure Report: Not required
