The Greater Los Angeles VA Healthcare Network is recruiting for a Senior Social Worker in the Inpatient Medicine/Surgery units and/or clinics for Social Work Service. S/he is a member of a multidisciplinary team whose goal it is to provide a wide range of health services to patients, as well as their families. The incumbent should be experienced in medicine and/or mental health, geriatrics and rehabilitation care, medical and mental health diagnoses, disabilities, and many treatment procedures. The Senior Social Worker will provide social work service on the inpatient medical/surgical wards and/or clinic area to include weekends and/or holidays. The wards include general medicine, surgical, intensive care, progressive care units, cardiology, neurology, cardiac and thoracic surgery clinics, acute rehabilitation and transitional care units. The major duties and responsibilities include but are not limited to: Performs case finding functions, identifying and screening patients for psychosocial needs, to include high risk screening. Develops treatment plans in collaboration with the Veteran/family and with the multidisciplinary team in a timely manner. Provides crisis intervention services, seeking to address the cause as well as the presenting complaint, coordinates family conferences and serves as liaison to family members. Assesses and treats the complicated psychosocial problems of Veterans, families and significant other. Provides assessment of psychosocial problems that cause distress, can include adjustment to the current medical condition and/or functional status, untreated or under-treated mental health or substance abuse condition, economic instability, legal problems, and inadequate social supports, housing and transportation. Coordinates with VA programs/staff and community agencies to assist with referrals as part of discharge planning. Establishes and maintains effective therapeutic relationships with Veterans and their families under their care in the hospital. Works with Veterans and their families who are experiencing a wide range of complicated medical, psychiatric, emotional, behavioral, financial, legal and psychosocial problems and documents after each significant contact and completes either Event Capture or an Encounter. Provides advanced level case management interventions and independently provides a full range of concrete services including, but not limited to, completing various forms and applications to ensure Veterans receives all benefits and services they are entitled to (i.e. nursing home placement, board and care, SSDI, and State Veterans Homes etc.). Aids in the procurement and retention of benefits, housing and where specialized services are required (i.e. legal issues, home care, shelter evaluation and referrals, financial, transportation, food, Polysubstance abuse, patient education) will assist the Veteran in learning about and linking to the appropriate resources they require. Develops and implements methods for measuring effectiveness of social work practice and services in specialty areas, utilizing outcome evaluations to improve treatment services.
