VA Puget Sound Health Care System is seeking a quality-focused, highly motivated nurse for the position of Patient Safety Manager (PSM). The PSM coordinates and integrates the multiple aspects of quality within the care directly provided by nursing, and across the care delivered by others in the health care system. The PSM is critical to the surveillance and coordination that reduces adverse outcomes for our Veterans. The Patient Safety Manager Registered Nurse (RN) is assigned to Patient Care Services in the VA Puget Sound VA HealthCare System, and located at the Seattle Campus. As QSV Patient Safety Manager, the RN manages and develops improvements for patient safety issues. The RN will travel and work on site at Seattle, American Lake campus and Community Based Outpatient Clinics when processing EOC rounds; OIG and The Joint Commission Surveyors on site when needed. Duties include but not limited to: Assists in development of program objectives and operating procedures and changes thereto. Works independently in prioritizing, planning, and determining methods to accomplish work assignments within the parameters of existing policies, procedures, and operating instructions. Responsible for the articulation of the Patient Safety Program; Root Cause Analysis (RCA), Healthcare Failure Mode and Effect Analysis (HFMEA) and adverse events. Assures effective implementation and education to staff on the Joint Commission Patient Safety Standards. Coordinates day-to-day activities of the Office of Quality, Safety & Cultural Transformation involving patient safety activities. Develops, assists others to develop practice guides and standards for continuous improvement in care. Manages/supervises maintenance of data, records, and correspondence related to the Patient Safety Program. Assumes leadership role and fosters accountability in others for patient safety, professional practice and improvement of patient care. Develops data collection tools, analyzes results, develops and distributes reports related to patient safety. Collaborates with interdisciplinary groups to identify areas for improvement for patient safety issues. Exercises judgment and ingenuity in evaluating health care operating programs and systems with regards to patient safety. Projects and prepares review schedules, conducts reviews, interacts with operating management. Develops strategies to support leadership in proactive planning to improve the patient safety program. Actively involved in evaluating practice for a program or service, based on established professional, community or regulatory standards (i.e. Joint Commission) and recommending/implementing changes to meet those standards. Maintains knowledge of current trends in nursing and health care administration and incorporates current concepts into practice. Designs/redesigns, implements, evaluates and collaboratively maintains the Medical Center's Patient Safety Program. Collects, monitors, analyzes, and reports patient safety data and collaborates with executive leadership and unit management to implement recommendations for performance improvement. Manages the patient safety reporting system; reporting trends, outliers, concerns, and appropriately referring reports for follow up. Assigns Safety Assessment Codes (SAC) to identified events Reviews recommendations by all RCA and HFMEA groups and assists leadership in assessing feasibility of these recommendations. Appropriately follows up on reported events, using the Patient Safety tools of Focused Review, RCA, local survey, audits, and facilitates problem solving and process improvement. Provides expert leadership and facilitation for Root Cause Analysis (RCA) and Healthcare Failure Mode and Effect Analysis (HFMEA) teams and other groups that have the objective of improving organizational patient safety. Ensures that RCAs and HFMEAs are complete, timely, technical requirements are met and all implementation requirements are accurately identified. Tracks completion of action items identified by RCA and HFMEA teams. Works with service chiefs to ensure timely completion of all proposed actions and outcomes and evaluation of their effectiveness. Demonstrates thorough knowledge of patient safety standards and requirements of external accrediting and regulatory bodies, e.g. Joint Commission, OIG etc. Work Schedule: Monday-Friday, 8:00 a.m.-4:30 p.m. Financial Disclosure Report: Not required Pay: Competitive salary, regular salary increases. Paid Time Off: Annual leave at the rate of eight (8) hours per biweekly pay period; Sick leave at the rate of four (4) hours per biweekly pay period; 10 paid Federal holidays per year.
