Housing and Urban Development/VA Supportive Housing is an intensive case management program for chronically homeless veterans aimed at providing psychosocial services to veterans while residing in housing through HUD's Section 8 voucher system. This position is responsible for case management and care coordination services for veterans receiving housing provided through HUD Housing Choice Program, and has a role across the continuum of homeless services from outreach to post housing services. Major Duties: Provide case management and care coordination for a caseload of veterans in the HUD-VASH program. Conduct psychosocial assessments to determine needs of veterans and family members. Provide individual, family and group counseling as appropriate. Act as a liaison between VA and community resources. Collect and submit HUD-VASH Program participant data as outlined by NEPEC evaluation procedures. Work Schedule: Monday - Friday, 8:00 AM - 4:30 PM Financial Disclosure Report: Not required Physical Requirements: Some physical duties including light carrying, moderate lifting, some walking, standing and possible kneeling; ability to operate, and get in and out of, various sizes of government vehicles from compact cars to multi-passenger vans; ability for rapid mental and muscular coordination; ability to distinguish basic colors.
