The clinical pharmacy technician for medication reconciliation is a certified pharmacy technician with the ability to support the medication reconciliation program. The primary functions of the Clinical Pharmacy Technician will spend majority of time providing pharmaceutical and clinical care for inpatient medication reconciliation. On-site certification of clinical competency shall be completed prior to performing assigned clinical functions. This position functions under the supervision of the assigned Clinical Pharmacy Specialists/Clinical Pharmacists, while directly reporting to the Clinical Pharmacy Manager of Acute Care. Other duties will be assigned and may include laboratory value monitoring, medical chart / action profile poly-pharmacy reviews and supplemental patient education communication. The Clinical Pharmacy Technician's responsibilities include: Interviews patients (and families/caregivers as appropriate), to develop an accurate list of medications, strengths and schedules. Compares inpatient, outpatient and non-VA medication lists in the medical record identifying and prioritizing the discrepancies. Contacts other pharmacies, hospitals or long term care facilities to obtain information related to most recent medication histories. Routinely obtains and reviews laboratory or medical information as assigned by the Clinical Pharmacy Specialist, and reports all values beyond normal limits to the Clinical Pharmacy Specialist for review. Make clinical assessments, document all clinical activities, update problem list, update non-va medication list and update medication allergy information in the medical record. Interview patients to ascertain significant medical & medication history/reconciliation. Evaluate medication regimens for appropriateness, drug interactions, therapeutic duplications, and adverse drug events. Assesses compliance with prescribed therapy through patient/caregiver interviews and review of refill dates. Provides educational information to patients as directed. Responsible for accurate and timely computer data entry. Enters accurate progress notes into the patient's medical record using computer templates and information obtained directly from the patient. Prioritizes operational and assigned clinical functions to provide the highest level of patient care. Reports all potential patient or pharmacy distributions problems and assists the Clinical Pharmacy Specialist/Clinical Pharmacist in reporting adverse drug reactions, medication errors, prescription non-adherence and near misses. Contributes to data entry related to quality assurance projects or other related projects. Assists with transport of medications to Clinical Pharmacy Specialists, Clinical Pharmacists, Nurses or Patients. Provides accountability for medication inventory tracking for all medications with special attention related to controlled substances. Confirms contents of medications leaving the outpatient pharmacy or being delivered to patients ensuring the correct patient name and expected medications to be present. Coordinates distribution of predefined workload to the Clinical Pharmacist/Clinical Pharmacy Specialists groups. Adjusts distribution if scheduled coverage is changed. Assists with processing reports, correcting/adjusting team assignments. Participates in team and committee meetings as assigned. Completes minutes for assigned meetings. Assists with administrative metrics related to consult gatekeeping, open encounters, pending consults, unsigned notes or other assigned duties. Work Schedule: 7:00 a.m. to 3:30 p.m. or 9:00 a.m. to 5:30 p.m. and occasional weekends and Holidays Financial Disclosure Report: Not required
