Our dynamic Fairview Home Care and Hospice Rehab team serves a diverse adult patient population. We are looking for a Supervisor with a degree in PT, OT, or Speech to support our collaborative team of health care professionals to improve patient outcomes and quality of life: patient focused and outcome driven.
1.0 FTE (80 hours in two weeks)
Locations: Central Metro - Minneapolis
Reports to Rehab Manager
Supervises and coordinates Rehab services in the Home Care and Hospice setting. Works with Rehab Manager, staffing department and human resources staff to assure adequate number and level of staff to meet patient needs. Assures quality care, regulatory compliance and customer satisfaction. May provide direct patient care, as needed.
Bachelor'sDegree or Masters Degree or Doctoral in Physical Therapy or Occupational Therapy; or Masters Degree in Speech Language Pathology
Minimum of 2 years direct patient care experience as a PT, OT, or SLP in home care/hospice
Current license in Physical Therapy or Occupational Therapy in the State of MN or current license in Speech Language Pathology in the State of MN and ASHA Certificate of Clinical Competence (CCC-SLP)by hire date
Current CPR certification (BLS for Health Care Providers)by hire date
Provide direct patient care as necessary
Must have reliable transportation
Current Minnesota drivers license (or license that matches your permanent residence)
Must carry current and adequate automobile insurance
Competent in the point of care computer system, EMR
