Provides personal care assistance to participants at the Adult Day Health Center (ADHC) and provides personal care, homemaker services, personal interaction and restorative care to participants in their homes to maintain a healthy and independent lifestyle in the community.
The Personal Care Assistant (PAC), works independently in a safe and courteous manner, under the direction of a care plan developed by Home Care Coordinator and Participant, to provide care needed, allowing the maximum level of independence in the home. The Personal Care Assistant is required to go to assigned interdisciplinary team and scheduled monthly meetings, and complete all required educational trainings.
The PCA reports directly to the Home Care Coordinator. In the absence of the Home Care Coordinator, the PCA reports to the Center Manager.
COMPETENCIES AND SKILLS:
Demonstrated personal care and homemaking. Demonstrated sincere interest in working with frail elderly participants is a must. Demonstrated good communication skills. Demonstrated ability to write and speak English. Demonstrated ability to follow directions without constant supervision. Demonstrated ability to work as part of the multidisciplinary team and make meaningful contributions to the development of the plan of care.
Performs duties in a self -directed manner with minimal supervision or direction. Ensures that routine and priority tasks are completed within established departmental time frames.
Basic computer skills preferred.
EDUCATION AND/OR EXPERIENCE:
Graduation from high school or equivalent. Certification of Home Health Aide and/or Nursing Assistant in the state of Pennsylvania is preferred.
Experience in personal care of an adult and homemaker service is required. One year of experience working with a frail or elderly population required. Care of older adults is preferred in direct and indirect personal care. Prefer Home Health or Skilled Nursing Facility experience.
Current CPR and First Aide certification. Must have certifications 6 months after date of hire. Valid Pennsylvania drivers license, reliable transportation with Auto insurance. Flexibility in the care of LIFE Geisinger participants needs. Initiative and imagination in implementing care to participants. Ability to communicate with participants and families in an effective, mature and caring manner.
WORKING CONDITIONS/PHYSICAL DEMANDS:
Work is typically performed in a home health setting or in the adult day health center. Frequent standing, walking, climbing stairs, lifting/carrying up to approximately 35 pounds and driving an automobile are involved in job duties. Occupational exposure to disease and illness including, but not limited to, Hepatitis B and HIV viruses involved in job duties.
