The Admission Nurse drives admissions by supervising the various steps in the referral to admission process by overcoming admission barriers and ensuring that all eligible patients who choose hospice are admitted to services. The Admission Nurse will also ensure a smooth referral and prompt patient/family visit to explain hospice services. This is an exempt position.
Essential Job Functions/Responsibilities
Supervises referral to admission process with close attention to length of intake, conversion ratio, and admission team productivity.
Determines monthly admission goals along with the Community Outreach Liaison.
Sets goals for increased conversion ratios (e.g. 80%). This should be done monthly.
Ensures same day admits of all referrals when possible. Length of intake = time of referral to admission. When the patient requires additional evaluation, a visit will be made by the Nurse Practitioner prior to discounting their eligibility.
Completes and submits timely admission documentation that is accurate, comprehensive, and reflective of the appropriate terminal diagnosis.
Participates in the orientation of all nurses and other IDT members who will be responsible for conducting the admission process.
Assists in the development of an admission or presentation guide for staff to streamline the admission presentation.
Follows up at appropriate time intervals with all referrals not admitted on a regular basis, at a minimum once monthly.
Educates everyone who answers phones. Equips them with the mindset and the tools to collect information from inquiry callers that may be potential referrals.
Acts as a backup RN for case managers when needed.
Promotes Hospice of Midland in the community.
Supports the Hospice of Midland Mission and Values.
Completes other projects and duties as assigned.

Must have a current Texas RN license.
Must be available to work a regular office hours but may be needed for a more flexible schedule at times.
One (1) year’s Hospice experience preferred.
Must have a thorough knowledge of the Hospice admission process.
Must have a working knowledge of Federal Regulations/Conditions of Participation as they relate to referral and admission process for Hospice.
Must be knowledgeable and stay up to date on Local Coverage Determinations.
Must have the ability to work well with physicians and facilities.
Basic computer skills.
Must be punctual, reliable and available during office hours and for on-call as required; duties may include regular hours outside of these.
Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order.
Organization personnel are expected to participate in appropriate education as may be requested by their immediate supervisor. In addition, organization personnel are expected to accept personal responsibility for other educational activities to enhance job-related skills and abilities. All personnel must attend mandatory educational programs.
The Admission Nurse must be able to sit, stand, squat, bend, kneel, twist and climb. They must also be able to lift and push/pull up to 50 pounds. They also occasionally are exposed to dust, walk on uneven or rough ground, and work near moving equipment. Hands will be used for repetitively grasping and turning, fine manipulation and speed work.
ENVIRONMENTAL AND WORKING CONDITIONS
May be exposed to extremes of heat and cold in all weather conditions. Must drive in various weather conditions on roads in varying degrees of repair.
May be exposed to infections and contagious diseases. May be exposed to patient elements. Subject to varying and unpredictable situations. Handles emergency or crisis situations. Travel required. OSHA Exposure Category I (position includes tasks that involve exposure to blood, body fluids or tissues).
