The Social Worker is responsible for the implementation of standards of care for medical social work services. This is an exempt position.
Essential Job Functions/Responsibilities
Assesses the psychosocial status of patients and families/caregivers related to the patient's terminal illness and environment and communicates findings to the Case Manager and other members of the interdisciplinary group. Provides an assessment in the patient's identified residence and assistance when this is not safe and another plan is required.
Carries out social evaluations, including family dynamics, caregiver abilities, communication patterns, high-risks for suicide, negligence or abuse, and plans intervention based on evaluation of the findings. Counsels patient and family/caregivers as needed in relationship to stress, and other identified coping difficulties. Provides crisis intervention when necessary.
Assesses for and educates interdisciplinary group on any special needs related to the culture of the patient and family.
Maintains clinical records on all patients referred to social work.
Educates patients and families on, and assists in, preparation of advanced directives.
Provides information and referral services for patients and families/caregivers regarding practical and environmental needs.
Provides information to patients and families/caregivers and community agencies.
Serves as a liaison between patients and families/caregivers and community agencies.
Maintains collaborative relationships with organization personnel to support patient care.
Maintains and develops contacts with public and private agencies as resources for patient and personnel.
Participates in the development of the individualized Plan of Care, involving the patient and family. Attends interdisciplinary group meetings, assisting the team in recognizing the effects of the psychosocial stresses on the symptoms of the terminal illness.
Assists physician and other team members in understanding significant social and emotional factors related to health problems and death/dying issues.
Actively participates in Quality Assessment Performance Improvement teams and activities.
Assists family and patient in planning for funeral arrangements, financial, legal, and health care decision responsibilities.
Assists in bereavement support for both Hospice of Midland and Rays of Hope as needed.
Administers the Pathways program, including development, implementation, participant visits and program evaluation. Maintains thorough and timely documentation of Pathways contacts. Regularly communicates and coordinates Pathways referrals to the IDT.
If the social worker is a licensed MSW, assists in the oversight of other Social Workers requiring supervision per the Centers for Medicare and Medicaid Services, HHS, §418.114(b)(3). This supervision includes documented oversight on a monthly basis, and will include evaluation of EMR documentation, overview with employee, and phone consultations as needed. Assists in the identification of in-service and continuing education topics to meet the needs of social services personnel.
Promotes Hospice of Midland in the community.
Supports the Hospice of Midland Mission and Values.
Completes other projects and duties as assigned.

A graduate of a bachelor’s program in social work accredited by the Council on Social Work Education; Master’s in Social Work (MSW) preferred. Must be licensed in the State of Texas.
Minimum of one (1) years’ experience in health care, hospice experience preferred.
Demonstrates good verbal and written communication, and organization skills.
Must be punctual, reliable and available during office hours of 8:30 a.m. to 5:00 p.m. Monday through Friday and for on-call as required; duties may include regular hours outside of these.
Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order.
Organization personnel are expected to participate in appropriate education as may be requested by their immediate supervisor. In addition, organization personnel are expected to accept personal responsibility for other educational activities to enhance job-related skills and abilities. All personnel must attend mandatory educational programs.
The Social Worker must be able to sit, stand, squat, bend, kneel, twist and climb. They must also be able to lift and push/pull up to 30 pounds. They also occasionally are exposed to dust, walk on uneven or rough ground, and work near moving equipment. Hands will be used for repetitively grasping and turning, fine manipulation and speed work.
ENVIRONMENTAL AND WORKING CONDITIONS
May be exposed to extremes of heat and cold in all weather conditions. Must drive in various weather conditions on roads in varying degrees of repair.
May be exposed to infections and contagious diseases. May be exposed to patient elements. Subject to varying and unpredictable situations. Handles emergency or crisis situations. Travel required. OSHA Exposure Category I (position includes tasks that involve exposure to blood, body fluids or tissues).
